Middletown sits about thirteen miles east of downtown Louisville, an incorporated city in Jefferson County along U.S. Highway 60. The area built up around a historic main street and grew outward through a series of subdivisions from the 1960s onward. Old Shelbyville Road runs through the original commercial core, while Shelbyville Road and the surrounding subdivisions hold most of the housing stock.

Middletown blends an older small-town main street with newer suburban subdivisions. The original core along Main Street holds a handful of older brick storefronts and a small set of historic frame houses. Surrounding the core, subdivisions built between the 1960s and the 1990s hold mid-century ranches, two-story Colonials, and split-levels on moderately sized lots. Newer subdivisions on the eastern edge extend the housing into the 2000s and 2010s. Interstate 64 and the Gene Snyder Freeway both touch the area, putting downtown Louisville about fifteen minutes away. Shelbyville Road runs east-west as the commercial spine, with shopping, restaurants, and offices along most of its length.
